Abstract Context: Established in 1997, Summa Health System’s Medical Ethics Committee (EC) serves as an educational, supportive, and consultative resource to patients/families and providers, and serves to analyze, clarify, and ameliorate dilemmas in clinical care. In 2009 the EC conducted its 100th consult. In 2002 a Palliative Care Consult Service (PCCS) was established to provide supportive services for patients/families facing advanced illness; enhance clinical decision-making during crisis; and improve pain/symptom management. How these services affect one another has thus far been (...) unclear. Objectives: This study describes EC consults: types, reasons, recommendations and utilization, and investigates the impact the PCCS may have on EC consult requests or recommendations. Methods: Retrospective reviews of 100 EC records explored trends and changes in types of consults, reasons for consults, and EC recommendations and utilization. Results: There were 50 EC consults each in the 6 years pre- and post-PCCS. Differences found include: (1) a decrease in number of reasons for consult requests (133–62); (2) changes in top two reasons for EC consult requests from ‘Family opposed to withdrawing life-sustaining treatment (LST)’ and ‘Patient capacity in question’ to ‘Futility’ and ‘Physician opposed to providing LST’; (3) changes in top two recommendations given by the EC from ‘Emotional Support for Patient/Family’ and ‘Initiate DNR Order’ to ‘Comfort Care’ and ‘Withdraw Treatment.’ Overall, 88% of recommendations were followed. Conclusion: PCCS availability and growth throughout the hospital may have influenced EC consult requests. EC consults regarding family opposition to withdrawing LST and EC recommendations for patient/family support declined. Young children, like adults, understand that human agents can flexibly choose different actions in different contexts, and they evaluate these agents based on such choices. However, little is known about children's tendencies to attribute the capacity to choose to robots, despite increased contact with robotic agents. In this paper, we compare 5‐ to 7‐year‐old children's and adults’ attributions of free choice to a robot and to a human child by using a series of tasks measuring agency attribution, action prediction, and (...) choice attribution. In morally neutral scenarios, children ascribed similar levels of free choice to the robot and the human, while adults were more likely to ascribe free choice to the human. For morally relevant scenarios, however, both age groups considered the robot's actions to be more constrained than the human's actions. These findings demonstrate that children and adults hold a nuanced understanding of free choice that is sensitive to both the agent type and constraints within a given scenario. (shrink)

Background Unit-based critical care nurse leaders (UBCCNL) play a role in exemplifying ethical leadership, addressing moral distress, and mitigating contributing factors to moral distress on their units. Despite several studies examining the experience of moral distress by bedside nurses, knowledge is limited regarding the UBCCNL’s experience. Research aim The aim of this study was to gain a deeper understanding of the lived experiences of Alabama UBCCNLs regarding how they experience, cope with, and address moral distress. Research design A qualitative descriptive (...) design and inductive thematic analysis guided the investigation. A screening and demographics questionnaire and a semi-structured interview protocol were the tools of data collection. Participant and research context Data were collected from 10 UBCCNLs from seven hospitals across the state of Alabama from February to July 2023. Ethical considerations This study was approved by the Institutional Review Board at the University of Alabama in Huntsville. Informed consent was obtained from participants prior to data collection. Findings UBCCNLs experience moral distress frequently due to a variety of systemic and organizational barriers. Feelings of powerlessness tended to precipitate moral distress among UBCCNLs. Despite moral distress resulting in increased advocacy and empathy, UBCCNLs may experience a variety of negative responses resulting from moral distress. UBCCNLs may utilize internal and external mechanisms to cope with and address moral distress. Conclusions The UBCCNL’s experience of moral distress is not dissimilar from bedside staff; albeit, moral distress does occur as a result of the responsibilities of leadership and the associated systemic barriers that UBCCNLs are privier to. When organizations allocate resources for addressing moral distress, they should be convenient to leaders and staff. The UBCCNL perspective should be considered in the development of future moral distress measurement tools and interventions. Future research exploring the relationship between empathy and moral distress among nurse leaders is needed. (shrink)

Moral distress (MD) is well-documented within the nursing literature and occurs when constraints prevent a correct course of action from being implemented. The measured frequency of MD has increased among nurses over recent years, especially since the COVID-19 Pandemic. MD is less understood among nurse leaders than other populations of nurses. A qualitative systematic review was conducted with the aim to synthesize the experiences of MD among nurse leaders. This review involved a search of three databases (Medline, CINAHL, and APA (...) PsychINFO) which resulted in the retrieval of 303 articles. PRISMA review criteria guided authors during the article review and selection process. Following the review, six articles were identified meeting review criteria and quality was assessed using the Critical Appraisal Skills Programme (CASP) Checklist for qualitative studies. No ethical review was required for this systematic review. The six studies included in this review originated from the United States, Brazil, Turkey, and Iran. Leadership roles ranged from unit-based leadership to executive leadership. Assigned quality scores based upon CASP criteria ranged from 6 to 9 (moderate to high quality). Three analytical themes emerged from the synthesis: (1) moral distress is consuming; (2) constrained by the system; and (3) adapt to overcome. The unique contributors of MD among nurse leaders include the leadership role itself and challenges navigating moral situations as they arise. The nurse leader perspective should be considered in the development of future MD interventions. (shrink)

A community's abilities to promote health and maximize its response to public health threats require fulfillment of one of the four elements of public health legal preparedness, the capacity to effectively coordinate law-based efforts across different governmental jurisdictions, as well as across multiple sectors and disciplines. Government jurisdictions can be viewed “vertically” in that response efforts may entail coordination in the application of laws across multiple levels, including local, state, tribal, and federal governments, and even with international organizations. Coordination of (...) legal responses to public health emergencies also may involve a horizontal dimension comprising numerous and diverse sectors, such as public health, public and private health care, emergency management, education, law enforcement, the judiciary, and the military.Although responses to many acute health threats can implicate multiple jurisdictions and sectors, the jurisdictional and sectoral dimensions of legal preparedness are complex and may vary substantially by the nature of a threat, its geographic and geopolitical extent, and the operational response demanded. The thymus gland, which plays a key role in the maturation and functioning of the lymphoid system, is implicated in the acquired immune deficiency syndrome (AIDS). The observation that the thymic hormone, thymosin α1, is elevated in individuals at risk for AIDS (as opposed to being depressed in other immunodeficient states) has provided the first direct evidence that the thymus is malfunctioning early in the course of this deadly disease. These observations have been valuable in screening for the syndrome with (...) a rapid radioimmunoassay and in the initiation of the first clinical trials with thymosin in high risk homosexuals and hemophiliacs. If the progressive immune paralysis in AIDS is due to a dying thymus, the early identification of asymptomatic carriers of AIDS or individuals with modest AIDS‐related dysfunction may lead to therapy with thymosin or other thymomimetic agents that can restore immune function and prevent the onset of frank AIDS. Human faces are one of the most prominent stimuli in the visual environment of young infants and convey critical information for the development of social cognition. During the COVID-19 pandemic, mask wearing has become a common practice outside the home environment. With masks covering nose and mouth regions, the facial cues available to the infant are impoverished. The impact of these changes on development is unknown but is critical to debates around mask mandates in early childhood settings. As infants grow, (...) they increasingly interact with a broader range of familiar and unfamiliar people outside the home; in these settings, mask wearing could possibly influence social development. In order to generate hypotheses about the effects of mask wearing on infant social development, in the present work, we systematically review N = 129 studies selected based on the most recent PRISMA guidelines providing a state-of-the-art framework of behavioral studies investigating face processing in early infancy. We focused on identifying sensitive periods during which being exposed to specific facial features or to the entire face configuration has been found to be important for the development of perceptive and socio-communicative skills. For perceptive skills, infants gradually learn to analyze the eyes or the gaze direction within the context of the entire face configuration. This contributes to identity recognition as well as emotional expression discrimination. For socio-communicative skills, direct gaze and emotional facial expressions are crucial for attention engagement while eye-gaze cuing is important for joint attention. Moreover, attention to the mouth is particularly relevant for speech learning. We discuss possible implications of the exposure to masked faces for developmental needs and functions. Providing groundwork for further research, we encourage the investigation of the consequences of mask wearing for infants’ perceptive and socio-communicative development, suggesting new directions within the research field. Aranguren sobre «puede» y «debe» en ética, aquí se plantea el significado de los principios de ética en la situación de retroceso democrático del siglo XXI. Ha sido analizada por la filosofía y la teoría política recientes. En los años sesenta, las críticas de D. Sternberger al modelo de polis, de política cívica en las obras de H. Arendt, se apoyaban en la imagen de la ciudad hundida. Pero las preguntas sobre (...) el alcance del realismo («puede») y el lugar de los principios («debe») siguen abiertas. ¿Ultra posse nemo obligatur? Aquella imagen de la polis puede ser reemplazada por otra algo distinta, la ciudad sumergida. Se encuentra en la literatura, sería también la metáfora de la necesaria reconstrucción de las políticas democráticas, con el «deber ser» como núcleo de la cultura cívica. Los principios éticos y, en especial, justicia como equidad será aquí la referencia en dos situaciones: (1) la política de salud pública, (2) los procesos de paz. En conclusión, el marco normativo cuenta o ha de contar, tanto en la paz como en la guerra. Decir «nunca más» es también una forma de recordar el papel crucial de los principios. (shrink)

Fifty years after the death of Edmund Husserl, the main founder of the phenomenological current of thought, we present to the public a four book collection showing in an unprecedented way how Husserl's aspiration to inspire the entire universe of knowledge and scholarship has now been realized. These volumes display for the first time the astounding expansion of phenomenological philosophy throughout the world and the enormous wealth and variety of ideas, insights, and approaches it has inspired. The basic commitment to (...) phenomenological concerns found in all this variety makes this collection a most signifi cant historical document. This second volume of the collection bears witness to a deliberate shift of attention from the earlier to the later phase of Husserl's reflections. We see how his issues - intersubjectivity, ethics, human encounter, the societal world, empathy, the sphere of the self, and the surpassing of dichotomies (bodylpsyche, etc.) - are now at the center of attention in the human sciences. By resorting to the spiritual autobiography of Mother Teresa of Calcutta, an important religious and cultural personality of the 20th century, the author tries to emphasise the aspects of political theology that defined her way of acting and thinking and to show how she understood the relationship between religion and politics. Topics like poverty, love, giving, peace, sacrifice or responsibility are presented as keywords in the understanding of a complex vision with interdisciplinary relevance, while the two levels of poverty, (...) namely the material one, often accompanied by physical pain, maladies and other types of suffering, and the spiritual one, that is not necessarily related to the lack of basic material needs and can be found even in developed countries like the United States of America or Japan, are seen through the lenses of political theology. The author shows that, by finding practical bridges between spirituality and politics and by using as sources of inspiration not only Christian thinkers, but also thinkers from other religions, like Ghandi, the Albanese militant accomplished her vocation and also offered important and perennial principles which can help in the understanding of the contemporary world and in finding solutions to some of its crises. Although her message is a sort of combination between theology and social life, the political accents, sometimes theoretical, at other times practical, are important and can be used today for the development of a sustainable discourse for both the theological and the political sphere. CONTRIBUTION: The article brings into attention a spiritual autobiography that was not valorised properly by the contemporary research and in the same time offers an inter-disciplinary approach with ecumenical value. This paper explores some alternative accounts of doxastic disagreement, and shows what problems each faces. It offers an account of doxastic disagreement that results from the incompatibility of the content of doxastic attitudes, even when that content’s truth is relativized. On the best definition possible, it is argued, neither non-indexical contextualism nor assessment-relativism have an advantage over contextualism. The conclusion is that conflicts that arise from the incompatibility (at the same world) of the content of given doxastic attitudes cannot be (...) accommodated by theoretical positions that allow for the compatibility (at the same world) of the content of different doxastic attitudes. John MacFarlane defends a radical form of truth relativism that makes the truth of assertions relative not only to contexts of utterance but also to contexts of assessment, or perspectives. Making sense of assessment-sensitive truth is a matter of making sense of the normative commitments undertaken by speakers in using assessment sensitive sentences. This paper argues against the possibility of making sense of such a practice. Evans raised a challenge to the coherence of relative truth. A modification of the challenge (...) can be given against MacFarlane’s revised views on assertion. The main objection to the relativist is that rational and earnest speakers are not bound by assessment-relative standards of correctness. (shrink)

The authors examined cue competition effects in young children using the blicket detector paradigm, in which objects are placed either singly or in pairs on a novel machine and children must judge which objects have the causal power to make the machine work. Cue competition effects were found in a 5- to 6-year-old group but not in a 4-year-old group. Equivalent levels of forward and backward blocking were found in the former group. Children's counterfactual judgments were subsequently examined by asking (...) whether or not the machine would have gone off in the absence of I of 2 objects that had been placed on it as a pair. Cue competition effects were demonstrated only in 5- to 6-year-olds using this mode of assessing causal reasoning. (shrink)

The age at menarche, body height and weight of the daughters of farmers, farmer1977, a time of economic development, a decrease in age at menarche (by 0.74 years) and a secular trend in body height (by 2.4 cm/decade) was observed. In 19772001, age at menarche decreased and body height increased by 0.28 years and 2.9 cm respectively. The percentage of families owning a car, freezer and video increased during this period. These last results are indicative of an improvement in living (...) conditions, but the villagers regard themselves as losers as a result of the political transformation (1989) in Poland. (shrink)

Islamic ethics in the light of Qurʼanic teaching and Hadith; Customs and practices.

This article has one aim, to reject the claim that negation is semantically ambiguous. The first section presents the putative incompatibility between truth-value gaps and the truth-schema; the second section presents the motivation for the ambiguity thesis; the third section summarizes arguments against the claim that natural language negation is semantically ambiguous; and the fourth section indicates the problems of an introduction of two distinct negation operators in natural language.

Este trabajo surge a partir de la investigación general de las técnicas hidropónicas teniendo en cuenta sus ventajas y desventajas para de esta forma poder encontrar aquel factor determinante a través de una comparación de técnicas hidropónicas que permitan clasificarlas y escoger la mejor opción que genere menos impacto ambiental negativo y demuestre ser más productivo en los entornos urbanos. Adicionalmente, un factor determinante en las ciudades es su espacio limitado por lo que la mejor opción también deberá incluir un (...) óptimo manejo del espacio que permita a casi cualquier individuo poder aplicarlos desde su entorno sin recurrir a excesivas modificaciones. Como principal resultado se escogió a la Hidroponía recirculante como método predominante por los excelentes resultados que se obtienen con relación a los demás, adicionalmente, este puede ser fácilmente aplicado en los ambientes urbanos por su versatilidad y buen manejo de recursos. Spór o dopuszczalność aborcji podtrzymują sprzeczne twierdzenia na temat statusu istoty ludzkiej pojmowanej jako osoba i stanowiącej część wspólnoty ludzkiej, która przestrzega pewnych norm dotyczących ludzi. Centralną normą jest tu zakaz zabijania ludzi. Przedstawiciele stanowiska rygorystycznego twierdzą, że w przypadku niepewności, każdy egzemplarz gatunku ludzkiego należy traktować jako bezwarunkowo upoważniony do nieprzerwanego istnienia biologicznego ; organizm ludzki staje się osobą i członkiem wspólnoty moralnej od chwili zapłodnienia oocytu przez plemnik; każdy przypadek rozmyślnego zniszczenia ludzkiego embrionu należy uznać za niewybaczalne zabójstwo. Przedstawiciele stanowiska liberalnego utrzymują, że nie należy wprowadzać ustaw, których nie wspiera konsensus ; organizm ludzki staje się członkiem wspólnoty moralnej w chwili narodzin; przypadki zakończenia życia nie–osób należy traktować podobnie do zabijania nie–osób, tj. biorąc pod uwagę hierarchię podmiotów i motywy osoby działającej. Niestety, spór o dopuszczalność aborcji zwykle nie bierze pod uwagę stanowiska milczącej większości, która wydaje się uważać, że organizm ludzki staje się członkiem wspólnoty moralnej wtedy, gdy jego centralny układ nerwowy podejmuje swoje funkcje, tj. ok. dwunastego tygodnia ciąży; organizmowi ludzkiemu, który jeszcze nie ma statusu osoby, należy przypisać specjalny status, który przewyższa status zwierzęcia, lecz nie jest tożsamy ze statusem osoby. Wzięcie tych okoliczności pod uwagę musi skutkować akceptacją dla zakończenia życia płodów ludzkich wtedy, gdy występują poważne po temu przesłanki. (shrink)

Das Buch legt in geschichtlicher Reihenfolge dar, wie sich herausragende Denker des Abendlandes Gott geistig und geistlich angenähert haben und wer für sie Gott im Wesentlichen war. So war für die antiken Philosophen Platon und Plotin das Göttliche vor allem das Gute, Schöne oder Eine. Als christliche Denker suchten dann im Früh- und Hochmittelalter Augustinus, Richard von St. Viktor und Thomas von Aquin auf je eigene Weise die Dreieinheit Gottes zu erklären. Am Beginn der Neuzeit endete die lebenslange Suche des (...) Nikolaus von Kues nach dem angemessensten Gottesbegriff schliesslich beim "Können selbst." In der Neuzeit erfuhr Teresa von Avila Gott als das Innerste ihrer Seele, während ihr Ordensbruder Johannes vom Kreuz Gott als lebendige Liebesflamme erlebte. In seinen "Meditationen" bestimmte Descartes Gott als das vollkommenste Wesen. Wiederum dem dreieinen Gott näherte sich Hegel mit seiner spekulativen Lehre vom Reich des Vaters, des Sohnes und des Geistes. Für Hans Urs von Balthasar war im 20. Jh. das stellvertretende Leiden des Gottessohnes für uns Menschen von entscheidender Bedeutung. Im selben Zeitraum verstand Karl Rahner Gott primär als das heilige Geheimnis. Anhand eines Hauptwerkes oder mehrerer Werke wird jeweils der Weg oder ein Weg dieser Denker zu Gott nachgezeichnet.
